This document lists various fill-out-forms related to CUPFA member activities, including Teaching Forms, Professional Development Forms, and Small Claims Forms. Please make sure that you have the appropriate reading/viewing software. See CUPFA Help for details.
It is critical that the Part-Time Teaching Application Form be filled completely and correctly and be submitted within the stipulated time frame. Before filling out the form, go through the Information Sheet carefully and, if you have any further questions, please contact us.
Note: This form supersedes the one included as Appendix H in the current Collective Agreement and should be used starting May 1, 2009. If someone is not aware of this change and uses the older form to apply, it will be accepted for the May 15, 2009 deadline. Thereafter, the new form must be used.
